Feedback Form

How to Create an Event Feedback Form in Zuke

The Community Sign Up Page feature in Zuke allows you to create a form that people can complete to provide feedback. You can use the AI assistant to generate the form, customise its questions, and configure settings such as the submit button label and banner image.

Step 1: Open the Community Sign Up Page
  1. Log in to the Zuke platform.

  2. Select the business you want to work with, for example, Your Marketplace.

  3. Navigate to Businesses.

  4. Click Community Management.

  5. Select Community Sign Up Page.

Step 2: Configure the Form
  1. Use the AI assistant to create your form.

  2. Enter a name for the form.

  3. Select Event as the form type, since you are creating an event feedback form rather than a newsletter or standard form.

  4. Add any specific instructions or details for the AI assistant. This field is optional.

  5. Choose whether the form should contain one page or multiple pages. For this example, select a single page.

  6. Click the option to generate the form.

Step 3: Add the Form Name and Description

Once the form has been generated, review its details and customise them as needed.

  • Form name: Enter a suitable name, such as Event Feedback Form.

  • Description: Add a short message explaining the purpose of the form. For example: “Thank you for being part of our event.”

Step 4: Customise the Form Settings
  1. Change the submit button label to suit your form. Examples include Sign Up, Register, Apply, or Submit. For this example, use Submit.

  2. If you want the form to be associated with an existing community, select the relevant community. This is optional.

  3. Select the appropriate option for allowing an embedded domain, if needed.

  4. Add a banner image if you want to customise the appearance of your form. This is optional.

Step 5: Review and Customise the Questions

The form includes fields that users must complete, such as their email address and full name. In the example, both fields are required.

You can also customise the questions to collect feedback from participants.

Example questions include:

  1. How would you rate your overall experience?

  2. What did you enjoy most about the event?

  3. What can we improve about your experience?

To customise a question, select the question field, choose the appropriate question type (such as text), and edit the question wording.

Make sure each question is marked as required only if users must answer it before submitting the form.

Step 6: Review Your Form

Before using the form, check that:

  • The form name and description are correct.

  • The submit button label is appropriate.

  • The email address and full name fields are configured as required.

  • Your feedback questions are clear and relevant.

  • Any optional settings, such as the community link or banner image, are configured as needed.

Step 7: Configure the Thank-You Message
  1. After completing the form questions, click Next.

  2. Review the thank-you message displayed to users after they submit the form.

  3. Edit the message if you want to customise it.

Step 8: Review the Confirmation Email
  1. Review the email that users will receive after completing the form.

  2. Check the email content and make any necessary changes, if editing is available.

  3. Confirm that the message is appropriate for your event feedback form.

Step 9: Test the Feedback Form
  1. Open the generated feedback form.

  2. Fill in the required details, such as your email address and full name.

  3. Answer the feedback questions.

  4. Submit the form.

  5. Check your email inbox to confirm that you received the confirmation email.

Testing the form helps you verify that users can submit their feedback and receive the expected email after submission.
